Parish Facility Request Form

This form is to be used for parish organizations looking to schedule an event.

Parish facilities are NOT available for rental purposes.

• Please complete and submit this form to request a room reservation.

• ​Once received, your request will be reviewed.

• Please wait for confirmation of availability before solidifying or promoting your event.

• Reservations are on a first come, first served basis.

• Set-up and clean up is the responsibility of the requesting organization.
